How to Complete the Diagnostic
Step 1: Evaluate Each Statement
Carefully read each statement and consider how accurately it reflects your organization’s current practices. Use the scale provided to rate each statement.
Step 2: Rate on a Scale of 1 to 5
Assign a score to each statement based on its truthfulness: 1 for not true, 3 for partially true, and 5 for completely true. This will help in identifying areas that need improvement.
Step 3: Analyze Your Score
After rating all statements, tally your scores to understand your organization’s role clarity. A higher score indicates stronger role clarity and accountability.

Quick Diagnostic Rule
If three or more questions score 3 or below, Role clarity is likely weakening execution.
40–50
Your structure is likely supporting EOS well.
Roles are reinforcing accountability and process discipline.
30–39
Some Role friction exists.
Your Accountability Chart may look clear but occasionally leaks.
20–29
Structural confusion is likely slowing execution.
Issues are probably rolling off the table or getting stuck between Roles.
Below 20
Your Accountability Chart is almost certainly leaking like a sieve.
Seat clarity exists. Role clarity does not.
The Final Test
Ask your leadership team this simple question:
When an issue appears, can we immediately answer:
- Which Role owns thinking about it?
- Which Role owns doing it?
- Which Role owns improving the process behind it?
If the answer is not immediately clear, the structure is incomplete.
Issues will continue to roll across the domed table and fall through the gaps between Seats and Roles.
Scoring Instructions
Evaluate Your Organizational Structure
To calculate your total score, add the ratings for each statement. The maximum possible score is 50. This score reflects the clarity and effectiveness of your role structure.
Interpret your results: A score of 40–50 indicates strong role clarity and accountability. Scores between 30–39 suggest some friction, while 20–29 points to structural confusion. Scores below 20 highlight significant issues with role clarity.
